Health maintenance organizations (HMOs) and preferred provider organizations (PPOs) have become the dominant form of health care delivery in the U.S. Managed care systems replace traditional fee-for-service medicine's incentives to over-treat with strong incentives to under-treat. This change in financial incentives creates a need to strengthen oversight to ensure delivery of timely, quality health care to all enrollees.
